Your First Visit Checklist: What to Bring to Your Annual Eye Health Consult

By Dr. Kenneth Wong, OD

Welcome to Michigan Eye Consultants

If you're coming to see us for the first time, welcome. We're accepting new patients, and I want your first visit to feel easy from the moment you walk in.

A few minutes of preparation goes a long way. When you bring the right things, check-in moves faster and I get a complete picture of your eye health. Here's what I ask every new patient to bring to their Annual Eye Health Consult.

Your First-Visit Checklist

  • Photo ID. A driver's license or state ID helps us set up your record correctly.

  • Medical insurance or Medicare card. Your Annual Eye Health Consult is billed to your medical insurance or Medicare, so this is the card we need most.

  • Vision plan information. If you might pick up new glasses, bring your vision plan details too. When you purchase glasses through us, we file a complimentary claim on your behalf using your vision benefits.

  • Your current glasses and contact lenses. Bring every pair you use, even ones you rarely wear. If you wear contacts, the box or a lens packet tells us your current brand and prescription.

  • A list of medications and eye drops. Include prescriptions, over-the-counter drops, and supplements. Many common medications can affect your eyes, and this list helps me connect the dots.

  • Notes on what you've noticed. Jot down any headaches, dry or tired eyes, blurry vision at a distance or up close, or trouble seeing at night. Small details you might forget in the moment often matter the most.

What to Expect During Your Visit

Your Annual Eye Health Consult goes well beyond reading letters on a chart. Here's how your first visit typically flows.

  1. Check-in. Our front desk team confirms your information and insurance. This is where your checklist pays off.

  2. Retinal imaging. We take a detailed photo of the back of your eye. It gives me a clear view of your retina, optic nerve, and blood vessels, and it gives us a baseline to compare against at future visits.
  3. Your consult with me. We'll check your vision and prescription and talk through anything on your notes. As part of every consult, I also evaluate the health of the tiny oil glands in your eyelids, which play a big role in dry, irritated eyes.
  4. Neurolens screening. Every comprehensive consult at our practice includes a Neurolens screening, which measures how well your eyes work together. It's part of our standard of care, not an add-on.
  5. Optical. If you need new glasses, our optical team will help you find frames that fit your face, your prescription, and your daily routine.
